Amnestic Dosing Error — Cell Block C
A misfiled amnestic solution (Class-B, intended for Subject TS-0012) was inadvertently administered via the environmental dispersal system in Cell Block C. Fourteen (14) personnel present in the corridor at time of dispersal were exposed. Symptoms were consistent with retrograde amnesia spanning approximately 72 hours prior to exposure.
OUTCOME //
All fourteen affected personnel underwent supervised recovery. Memory gaps were assessed and documented. Three personnel experienced persistent discontinuity in procedural memory and were reassigned to non-sensitive posts pending further evaluation. The dispensing officer was placed on administrative suspension. Updated dispensary verification protocols (Directive 4.3-C) were implemented within five (5) days.

Standard Containment Breach — Cell Block A, Unit 7

The TW proceeded into the Cell Block A corridor before triggering secondary motion sensors. Response Team Alpha was mobilised within four (4) minutes.
OUTCOME //
TW-031 was located in the Cell Block A junction corridor and recontained without requiring deployment of enhanced restraint apparatus. Total breach duration: three (3) hours, forty-seven (47) minutes. Lock failure was assessed as mechanical rather than anomalous. Replacement hardware installed within 24 hours.

TW-013 Partial Corridor Escape — Sector 4 North Passage
TW-013 achieved partial egress from its designated containment area at 22:07 and proceeded along the Sector 4 North Passage for an estimated [REDACTED] metres before detection. The nature of TW-013's anomalous properties complicated standard recontainment approaches. Director S-0001 was notified at 22:19.
OUTCOME //
Recontainment was achieved at 23:41 following deployment of [REDACTED] countermeasures under emergency Directive 2.7. The North Passage was sealed for seventy-two (72) hours for decontamination and structural assessment. All personnel present during the event were administered Class-B amnestics as a precautionary measure and debriefed under Level-3 protocol.

TW-222 Resonance Event — Sector 7 Wing B
At 01:54, an unscheduled resonance event was registered originating from TW-222's containment chamber. All blast doors in Sector 7 Wing B engaged automatically under Directive 2.14. Sector-wide communications were disrupted for eleven (11) minutes. One (1) staff member who was within the corridor adjacent to the chamber at time of event is listed as [REDACTED] in personnel records.
OUTCOME //
Sector 7 Wing B remained under full lockdown for seventy-two (72) hours. The Stillwalk Protocol was successfully maintained throughout. TW-222 showed no deviation from its documented containment posture at any point during or after the event. Co-Founder S-0002 personally conducted the post-event assessment. TW-025 Expansion Event — Filing Room Perimeter
Routine dimensional survey of TW-025 (The Filing Room) detected an anomalous perimeter expansion of [REDACTED] cubic metres overnight between 2020-03-04 and 2020-03-05. The expansion was not accompanied by any structural breach of surrounding infrastructure. Surveying personnel reported the interior filing arrangement had altered significantly, though no existing files appeared to be missing.
OUTCOME //
No personnel entered the expanded section. A perimeter marker was placed at the new boundary. The expansion has not reversed as of this filing. Head of Subsurface Operations logged the event as 'ongoing — unresolved' and escalated to Director S-0001. No corrective action has been taken. It is noted in the Director's addendum that attempting to reverse an expansion of TW-025 may not be advisable.

External Intrusion Attempt — Tram Hub East Access
At 19:42, perimeter sensors at Tram Hub East Access registered an unauthorised entry attempt by an individual presenting a compromised cover identity credential. The individual was apprehended within the secondary airlock before reaching the interior. Preliminary assessment indicated awareness of M.A.L.I.C.E's existence at an operational level, suggesting possible intelligence leak from within the organisation.
OUTCOME //
The individual was detained and subjected to Level-3 assessment. All information gathered was forwarded to Co-Founder S-0002. The cover identity system employed at Tram Hub East was suspended and replaced within 48 hours. An internal security audit was initiated. Results are classified above Level-2.
NOTES //
The identity of the individual and the outcome of their detention are classified above Level-3. This entry is filed for incident logging purposes only. Twelve (12) personnel with Tram Hub access credentials were subsequently investigated under Directive 3.6.
TW-001 Sight Event — Unauthorised Visual Contact, Level-1 Personnel
Three (3) Level-1 clearance personnel reported visual contact with TW-001's golden light manifestation during a scheduled transit through the Sector 4 junction corridor. Contact duration was estimated at four (4) to seven (7) seconds per individual. Reporting was immediate and consistent across all three accounts. All three individuals are now classified as Golden Sight subjects.
OUTCOME //
All three personnel were assessed by the Golden Sight Assessment Unit within two (2) hours of the event. They were administered Class-B amnestics covering the specific incident and the preceding twelve (12) hours. Their ongoing status has been recorded on the Golden Sight Register. The full implications of their designation have not been disclosed to them in accordance with standing containment instruction. The corridor access route has been permanently restricted above Level-3.

TW-032 Anomalous Property Shift — Ongoing Monitoring Protocol
TW-032 exhibited a sudden and unexplained shift in its primary anomalous property on 2021-07-14 at approximately 11:30. The nature of the shift is classified above Level-2. Prior to this date, TW-032's anomalous behaviour had remained stable for [REDACTED] years without variation. Research staff noted the change during routine observation and immediately escalated.
OUTCOME //
Containment integrity was assessed and confirmed. No breach was recorded. TW-032's revised anomalous behaviour has been logged under a new containment sub-classification. Ongoing observation is in effect under a dedicated monitoring team. The Director has reviewed the case file personally. The cause of the shift has not been determined.

Seismographic data indicated a point source at a depth of [REDACTED] metres below the facility's lowest level — below any known M.A.L.I.C.E infrastructure. The event was preceded by a three (3) second acoustic phenomenon described by S9 personnel as 'structured,' though no definitive linguistic or signal content has been confirmed.

The affected junction was sealed under emergency protocol. Structural integrity was assessed as uncompromised. A secondary survey was dispatched and returned without reaching the source point.
