TW-118 cannot be contained. M.A.L.I.C.E maintains a global alert network to identify new manifestations. When located, a perimeter is established and descent is conducted under strict time protocols — maximum 90 minutes, mandatory return regardless of depth reached.
TW-118 is a structure of unknown origin that appears at a different location globally every 28–35 days. It manifests as a building indistinguishable from surrounding architecture but always empty, always unlocked, and always containing a single stairwell that descends without end. No floor has ever been reached by any descent team. The deepest confirmed descent is 340 metres below ground level before communications failed. The stairwell shows no signs of construction — walls are seamless. When the building dematerialises, all personnel inside are expelled to the nearest populated area with no memory of the descent.
TW-118 was first identified when three separate descent incidents across two years were cross-referenced and found to describe identical stairwell architecture despite occurring in different countries.

Descent team reported hearing a sound on approach to the 340-metre point — a steady rhythmic tone, like a very large, slow heartbeat. This was the furthest any team had descended. Communications failed at this depth as expected. Team was expelled after 89 minutes. All four members requested immediate transfer.
